The Georgia Hospital Association has recognized Candler County Hospital as a Gold Honor Roll member of the Health Improvement Innovation Network (HIIN) for mid-year 2018.
What is HIIN?
Hospital Improvement Innovation Network (HIIN) is a nationwide effort to reduce preventable hospital-acquired conditions and hospital readmissions. Addressing health equity for Medicare beneficiaries and incorporating personal and family engagement in health care is central to achieving this goal. HIIN works with local, state and national partners to continue the momentum of initiatives already working well and helps to distribute that information to other hospitals and health systems to duplicate the success.
The HIIN goal for 2018 has been to reduce all-cause inpatient harm or preventable conditions by 20 percent and reduce unnecessary readmissions by 12 percent. The Georgia Hospital Association (GHA) follows these initiatives at the state level and provides feedback and support to 97 participating hospitals in the state of Georgia. At the start of this initiative, Candler County Hospital was number 96.
Jeanne Blevins, CNO and her team at Candler County Hospital (pictured) attended the GHA HIIN Mid-year review meeting held last week in Dublin. During this meeting, it was announced that Candler County Hospital now ranks 6th in the state, landing CCH on the honor roll of the state’s Gold List! The measure of achievement follows 16 initiatives used by critical access hospitals to reduce patient harm, improve patient safety, prevent infection, educate on medication, fall prevention and adverse drug reactions. Candler County Hospital has worked to educate staff, patients and their family members on these initiatives and important topics related to health and safety. The work is really making a difference in the lives of our patients and through GHA, CCH is sharing best practices that can help other hospitals realize success as well. Now that’s really something to crow about!
